Spend the hour with the client, not with the paperwork.
Intake, the treatment plan, every session note and the measures that show progress live in one record — and your private process notes live somewhere else entirely.
- Signed BAA
- Encrypted and audited
- Assisted import
Notes written after dinner
DAP, SOAP and BIRP templates take the session in the structure your payer or supervisor expects, and they autosave as you type.
Process notes mixed into the chart
Psychotherapy notes are stored in a separate encrypted table that never appears in the chart, the client portal, prints or exports.
Outcomes you cannot show
PHQ-9, GAD-7 and PCL-5 are scored with severity bands and trended, so progress toward the plan is a chart rather than an impression.
Psychotherapy notes that HIPAA treats differently, handled differently.
Under 45 CFR 164.501 these notes sit apart from the medical record and are excluded from a client's right of access. Only the author, or someone you grant that one specific permission, can open them, and every open is logged.
- Wildcard admin roles never unlock them
- No export path at all, by design
- Session times and modality stay in the progress note, where they belong
Treatment plans, groups and safety plans built in.
Problems become goals, objectives and interventions with review dates and the client's signature. Run a group once and each member gets their own draft note in their own chart.
- Collaborative safety plan and discharge summary templates
- Guardians and consent to treat for clients under 18
- ABA session notes with BCBA and RBT roles
- Free 15-minute consultation as a bookable call
Configured for a counseling practice
Session lengths, clinician roles and every behavioral template are set up from the services you choose.

What we deliberately leave out
Some popular instruments, including the C-SSRS and the EPDS, carry their own licensing terms, so they are not bundled. Your clinicians can still document them as free text.
Coming from another system?
Therapists usually arrive from a practice-management suite built for counseling. Clients, diagnoses and appointment history come across; private process notes stay with you.

Questions
Can a pre-licensed associate use THNK Med?
Yes. Associate clinicians have their own role, and you decide exactly what that role is allowed to see and sign.
Do clients fill out measures before the session?
They can. A one-time link opens the questionnaire on the client's phone, and the score is waiting on the chart when the session starts.
Can I book couples and family sessions?
Yes. Couples and family therapy are their own 50-minute session types, next to 38- and 53-minute individual sessions and 90-minute groups.
Does psychological testing fit on the schedule?
Psychologists get a 180-minute testing block and a separate feedback session, so a full battery is not squeezed into ordinary therapy hours.
